At Infiniti Medical Clinic, many medical visits are covered under the Alberta Health Care Insurance Plan (AHCIP). However, some services are not included.
These uninsured services are not covered by AHCIP and may require out-of-pocket payment.

- Disability Benefit Reports
Work & School Forms
The following are uninsured services:
- Notes for illness or fitness
- Pre-employment medical exams and reports
- Pregnancy leave and Employment Insurance (EI) forms
Medical Fitness Forms
These uninsured services may include:
- Disabled parking authorization
- Driver’s medical forms
- Veteran Affairs forms
- Notes for camp, daycare, or travel
Government Forms
Completion of government-related documents is also categorized under uninsured services, including:
Alberta Programs
- AISH (Assured Income for the Severely Handicapped) forms
- Medical reports for work capacity
- Foster/adoptive parent medical references
- Handicapped Children’s Services documentation
Government of Canada
- CRA Disability Tax Credit forms
- CPP Disability (initial and reassessment reports)
