Visit Your Family Doctor
Describe your headache symptoms and ask for a referral to Infiniti Medical Headache Clinic.
Step 2
Referral Submitted
Your physician faxes a completed referral to 403-460-4569. Download our referral form below.
Step 3
Clinic Review
Our team reviews the referral and confirms our clinic is the right match for your care needs.
Step 4
Appointment Scheduled
We contact you directly to book at a convenient time.
Step 5
Your Treatment Plan
A comprehensive assessment and individualized care plan at your first visit.

How to Request a Referral
If you’re experiencing chronic headaches or migraines and haven’t found adequate relief through general care, you can access our clinic through your family physician or walk-in clinic doctor.
What to tell your doctor:
Describe Your Symptoms
Frequency, duration, intensity, location, and associated symptoms (nausea, light sensitivity, aura).
Share Your Treatment History
What you’ve tried, what’s worked, what hasn’t — including over-the-counter and prescription medications.
Ask Specifically for a Referral
Ask your doctor to refer you to “Infiniti Medical Headache & Migraine Clinic” in NW Calgary, or to fax the referral form to 403-460-4569.
